Somaliland is currently in a phase of epidemiological transition where population growth, urbanization, ageing, and improved standards of living will soon contribute to a growing burden of disease from NCDs. In a recent study, 18.2% of patients visiting hospitals in Hargeisa and Burao suffered from NCDs, with HTN and DM accounting for most cases at 13.7% and 7.5% of total visits, respectively (THET 2022). The World Bank (2020) reported that over 30% of the total deaths in Somaliland are attributed to NCDs, and all of these deaths occur prematurely.
With this background in mind, Pharo Foundation is implementing a pilot program on the prevention and control of non-communicable diseases (NCDs) especially diabetes and hypertension in Somaliland. The foundation supported the construction of a centre for NCDs in the country to provide high quality and effective clinical and preventive services to the community. The centre is located within Hargeisa Group Hospital (HGH).
Position Summary
We are looking for a Nurse, who is a specialist in Diabetes and Hypertension. The Incumbent will act as a lead nurse for the multidisciplinary team placed at the centre for Non-communicable Diseases (NCD centre) within Hargeisa Group Hospital (HGH). She/he will ensure the ongoing development and delivery of a high-quality service to patients. This will include the management of a case of patients, providing specialist advice within their clinical area for patients with complex diabetes needs including history taking, clinical assessments, risk reduction strategies and complication
prevention, and health education. The nurse is a key for the provision and continuity of care to improve quality of life for patients. post holder facilitates patient care by liaising with the multidisciplinary team and guiding best practices. She/he will also provide support and education for patients and their families.

Key Duties and Responsibilities
- As a practitioner nurse, deliver evidence-based care, structured education, and information to patients and their families.
- Assist in development and implementation of appropriate pathway of care for patients with NCDs especially diabetes and hypertension.
- Support, manage, monitor, and maintain the efficient and continued development of care within the center.
- Support clinical practitioners to ensure the consistent delivery of high-quality care within the center.
- Act as educator, and source of support to provide high-quality care, ensuring patients are appropriately clinically triaged and signposted to appropriate clinicians.
- Diagnosing and treating diabetes and its related complications by analysing, examining, and investigating patients and their medical histories for signs and symptoms of diabetes and hypertension.
- Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
- Assess, diagnosis, plan, implement, and evaluate interventions/treatments for patients with complex needs.
- To provide patients with patient-centred, evidence-based care and treatment according to local protocols, and guidelines.
- To prevent and detect early complications and other physical, social, or psychological problems.
- To educate patients, and relatives/cover all aspects of Diabetes and hypertension management by direct contact individually or in groups, within the practice.
- To set up and organise patient education clinics/sessions.
- Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role are maintained.
- To liaise effectively with other health professionals both inside and outside the centre, regarding accepting and making clinical referrals.
Skills and qualifications
Qualification Requirements
- Bachelor’s degree or master’s degree in nursing.
- Qualified and Practicing nurse.
- NHPC registered practitioner.
- Recent and relevant experience of working with patients with diabetes and hypertension.
- Experience in treating and managing patients with diabetes/hypertension and its complications.
- Relevant experience in the management of long-term conditions of up-to-date skills, knowledge, and understanding; and ongoing competence in Diabetes.
- Counselling skills/qualifications.
- Excellent decision making, communication skills as well as interpersonal and cross-cultural skills.
- Experience in supporting and educating patients to manage their health needs.
- Understanding evidence-based practice.
- Excellent time management skills and proven ability to prioritise workload.
- Experience in using Microsoft Word and Excel.
- Ability to work in a team with colleagues in own and other disciplines (as an essential).
- Able to engage and communicate with individuals and groups, both on a face-to-face basis and in writing.
- Agreeable to assisting in research, publications, and presentations.
- Fluent in Somalia and good verbal and written command of English.
