Re-advertisement: Invitation for Open Tender

Supply and Delivery of Medical Equipment to Lasanod General Hospital under PROSPERIS Program Funded by Royal Norwegian Embassy and Swiss Development Cooperation

Date: 24.03.2024.

NIS is looking for qualified supplier to undertake supply and delivery of medical equipment to Lasanod General Hospital. All interested suppliers are requested to submit their applications as the below requirements

- Licences:

o Valid registration from relevant government authorities/bodies such as Ministry of Commerce and Industry of FGS/FMS.

- Tax compliance certificate: A valid tax compliance certificate (TIN) from Ministry of Finance FGS/FMS.

- Proof/evidence of supplier ownership: all suppliers shall submit a signed public notary document stating the ownership of the supplier including names, positions/titles, contacts details and shares owned.

- Previous similar experience of supply and delivery: Shall submit ONLY 3 previous similar supply and delivery of medical equipment related contracts implemented with and/or for government institutions, International non-governmental organizations (INGOs) or UN agencies in the last 5 years. Submission of forged contracts will lead to automatic disqualification of the company.

The previous related contracts should:

Be signed and stamped contracts by both parties and not purchase orders.

Include itemized list of equipment with the specifications and

cost purchased and delivered.

Please fill the below table with the details of the three contracts being submitted (Note that only this template shall be applied when filling)

-         Suppliers Finances: Shall present a dully signed financial/bank statement confirming that the supplier is financially in good position and able to pre-finance the activity as may be necessary.

-         Delivery time: The supplier shall submit a clear and detailed delivery timeline.     

-         Price responsiveness: The supplier shall submit a filled and stamped quotation in both excel and PDF format.

-         Please note the successful bidder will be required to provide maintenance in case of any defects and/or technical failure in the future (one year).

-          Supplier ethical standard form: Fill, stamp and submit the attached supplier ethical standard form.
